Introduction

When a doctor says "your child needs a bone marrow transplant", most parents hear two things at once: a ray of hope and a very big number. A bone marrow transplant (BMT), also called a stem cell transplant, is the only established cure for thalassemia major. But it is expensive, and many families do not know that help exists.

This guide explains, in simple language, how much a BMT can cost in India, which government schemes and organisations may help, who is eligible, and the step-by-step process to apply. If you are looking for blood or want to understand the disease itself, you can also read our complete guide to thalassemia in India.

Important note:

Scheme rules, hospital lists and amounts change. Please confirm the latest details on the official portal or with the hospital before you apply. This blog is for information only and is not medical or legal advice.

How much does a BMT for thalassemia cost in India?

The cost depends on the hospital, the child's age and weight, the type of donor and whether complications arise. A news report on a new BMT facility in Andhra Pradesh quoted a range of roughly 8 lakh to 40 lakh rupees for BMT in India. A Health Ministry document on the Coal India transplant programme said that a low-risk transplant in a young child without major complications costs about 10 lakh rupees, but the bill can be higher in 30 to 40 percent of patients. The same document said that the two years after the transplant can need another 1 to 2 lakh rupees for medicines and tests.

So when you plan, ask the hospital for a written estimate that includes the pre-transplant tests, donor tests, hospital stay, medicines, and follow-up care. Also plan for travel and living costs near the hospital.

Low-Risk Young Child ~₹10 Lakh

Baseline in Coal India Health Ministry documentation for uncomplicated pediatric cases.

Standard National Range ₹8L - ₹40L

Varies by centre, donor match type, conditioning protocol, and complications.

Post-Transplant (2 Yrs) ₹1L - ₹2 Lakh

Essential follow-up, immunosuppressive medications, blood work, and checkups.

Who is the best candidate for a funded BMT?

Doctors and schemes generally prefer transplants that happen early, before iron overload damages the liver and heart. The Health Ministry document says that children up to 7 years of age who have been well chelated have success rates above 90 percent, while older children also do well, with success rates around 75 to 85 percent when carefully managed. It also notes that the scheme gave preference to children under 7.

Most funded programmes need a fully matched family donor, usually a brother or sister. So the first practical step is HLA typing of the patient and siblings.

Clinical Success Rates (Health Ministry Data)

Children up to 7 years (Well-chelated, low iron overload) > 90% Success
Older children (Carefully managed protocol) 75% - 85% Success

SCHEME 1: Thalassemia Bal Sewa Yojana (TBSY)

What it is. TBSY is the main financial aid scheme for thalassemia BMT in India. It was launched in 2017 and is run by the Ministry of Health and Family Welfare with funding from Coal India Limited's CSR programme. Eligible patients can get up to 10 lakh rupees for a transplant. The money is sent directly to the transplant hospital and not to the family.

Important detail: 10 lakh is the maximum assistance. It is not an unlimited reimbursement of every medical expense. If the total bill is higher, the family or hospital may need to find other funds.

Who is eligible. Eligibility has been updated across the phases of the scheme, so read the current rules on the portal. The following points come from the scheme's official documents and public information pages:

  • Thalassemia patients up to 12 years of age, and aplastic anaemia patients up to 18 years, according to the government information page on Vikaspedia.
  • The original programme guidelines required annual family income of 5 lakh rupees or less, with an income certificate from the competent authority.
  • The patient should have transfusion-dependent thalassemia and a matched related donor. In the original guidelines, unrelated donor, cord blood and half-matched (haploidentical) transplants were not covered, but later public information says rules differ for thalassemia, matched unrelated donor and aplastic anaemia cases, so check the latest rules.
  • The patient is assessed by the transplant centre, and the original guidelines excluded patients in the highest-risk class (Class IIIB).
  • Meeting the conditions does not by itself guarantee funding. Each case is examined before approval.

How the money is paid. Under the original guidelines, an initial 5 lakh rupees was sent to the transplant centre after the pre-transplant evaluation, and the balance was paid after the hospital sent the bills. The guidelines also said the transplant should happen within one to two months after the patient is allotted to a centre, and not later than three months after referral.

Where to apply. In May 2023 the Health Ministry launched an online TBSY portal on which families can track their application from registration to closure. According to the Vikaspedia page, eligible families can apply online at coalindia.in/tbsy. Applications can also be initiated through participating hospitals and through Thalassemics India, the coordinating organisation.

Hospitals. In the first two phases the scheme ran with 10 empanelled hospitals, and it later expanded. Hospitals named by the government information page include Narayana Hrudayalaya in Bangalore, CMC Ludhiana, Tata Medical Centre in Kolkata and Indraprastha Apollo in New Delhi, and AIIMS and Rajiv Gandhi Cancer Institute have also been reported. A recent policy summary says the network has grown to 21 hospitals and that more than 1,050 transplants have been completed. Because the list changes, always use the current "Selected Transplant Centres" list on the official portal, and do not rely on old lists on third-party websites.

SCHEME 2: Ayushman Bharat PM-JAY and PMJAY-MA Yojana (Gujarat)

In Gujarat, Ayushman Bharat PM-JAY works together with the state's Mukhyamantri Amrutum scheme under the combined name PMJAY-MA Yojana, which was integrated in 2019. According to the Gujarat Cancer and Research Institute, the cover is 10 lakh rupees per family per year for Gujarat beneficiaries, and treatment is cashless at empanelled hospitals across India. News reports say over 1.2 crore families in Gujarat are covered, with more than 2,000 empanelled hospitals.

Does it cover thalassemia BMT? A reply in the Indian Parliament said that treatment for anaemia and related complications, including bone marrow transplant and blood transfusion, is available in the Health Benefit Package list. However, coverage for a specific transplant depends on the package, the hospital and pre-authorisation, so do not assume. Ask the hospital's Ayushman Mitra (help desk) to confirm in writing whether your child's BMT and the associated costs can be approved under your PMJAY-MA card.

How to use it. Check that your family has a valid PMJAY-MA card. Ask the empanelled hospital to check the package and send the pre-authorisation request. Helplines listed by GCRI for PMJAY are 14555 and 1800 233 1022.

Support for tribal families in Gujarat

A state scheme reported by the media provides extra help for Scheduled Tribe patients in Gujarat who hold a PMJAY or MA card. It covers the remaining cost after the PMJAY amount has been deducted, and a valid caste certificate is needed. Check the Tribal Development Department for current rules.

SCHEME 3: Other government help

State programmes. Some states run their own transplant support. For example, Bihar has started a programme offering free BMT to children with thalassemia through sibling-matching camps, as reported in the press. Ask your state health department or district hospital whether your state has something similar.

Approach your District Chief Medical Officer of Health (CMOH) or Civil Hospital Helpdesk.

National Health Mission. Thalassemia care, including screening and blood services, falls under national health programmes, and your district hospital or civil hospital can tell you about free blood and medicines available locally.

Rashtriya Arogya Nidhi, Prime Minister's and Chief Minister's relief funds. These are one-time government assistance routes for serious illness. Rules, limits and eligibility change, so ask the hospital social worker or the concerned government office for the current process. These are mostly for patients being treated in government hospitals.

NGOs and community support

Thalassemics India. This national organisation coordinates the TBSY programme. Under the programme guidelines, applications are processed by thalassemia societies and state health departments and then sent to Thalassemics India, which forwards eligible cases to a central screening committee. Contacting your local thalassemia society is therefore one of the best first steps.

Local thalassemia societies and charitable trusts. Many cities have societies that provide free or subsidised blood, chelation medicines and guidance on applications. Ask your treating hospital for the nearest one. If you are in Gujarat, our team at LifeSavers United can also help you connect with blood donors and local support via our Doctors Network and Blood Information Center.

Donor registries. If no sibling matches, a doctor may suggest a matched unrelated donor. Registries such as DATRI help find volunteer stem cell donors. Note that some schemes cover only matched family donors, so ask about funding before choosing this route.

Crowdfunding Caution & Best Practices

Platforms such as Ketto and Milaap are used by many families. Use only the hospital's official estimate, share real documents, give regular updates and never pay anyone who promises "guaranteed approval".

Step-by-step: how to apply for BMT financial aid

1

Step 1. Confirm the diagnosis

Keep the HPLC or genetic report, transfusion history and recent reports for ferritin, liver function and heart evaluation.

2

Step 2. Consult a BMT specialist early

Ask whether your child is a good candidate and whether to do HLA typing for the siblings. Do not wait for complications.

3

Step 3. Do HLA typing

Test the patient and the siblings (and parents if the doctor advises). A fully matched sibling opens the most funding options.

4

Step 4. Choose a transplant centre

Ask whether it is empanelled under TBSY and accepts PMJAY-MA. Check the current list on the official portal.

5

Step 5. Get the cost estimate in writing

Ask for a breakdown of what is included and what is not.

5 Crucial Questions to Ask the Hospital MSW / Ayushman Mitra

Every empanelled hospital has a Medical Social Work (MSW) department and an Ayushman Mitra help desk. Take a notebook and ask these exact questions during your initial meeting:

  1. Scheme Empanelment Status: "Is this hospital actively performing BMTs under Coal India TBSY Phase 3, and is the patient quota currently open?"
  2. Combining Financial Aid: "Can we combine our PM-JAY / PMJAY-MA card with the Chief Minister Relief Fund (CMRF) or hospital charitable trust subsidies to cover any remaining gap?"
  3. Pre-Transplant Out-of-Pocket Expenses: "Which pre-transplant workups (such as high-resolution HLA typing, infection screens, or dental clearance) must be paid out-of-pocket, and can any part be reimbursed?"
  4. Post-Transplant Drug & Isolation Coverage: "Does the approved financial sanction cover post-discharge immunosuppressive medicines, CMV monitoring tests, and outpatient consultations for the first 6 to 12 months?"
  5. NGO & Registry Tie-Ups: "Does the hospital work with partner trusts or registries (such as Sankalp India Foundation, DATRI, DKMS-BMST, or Thalassemics India) that offer matching grants for shortfall expenses?"
6

Step 6. Collect your documents

Typically these include:

  • Child's Aadhaar or birth certificate and parents' identity proof
  • Address proof and ration card, if available
  • Income certificate from the competent authority
  • Thalassemia diagnosis and transfusion records
  • HLA typing report of patient and donor
  • Hospital's BMT recommendation and cost estimate
  • PMJAY-MA card, if you have one
  • Bank details of the parent (for non-scheme help such as relief funds)
  • Photographs and any extra documents requested during scrutiny
7

Step 7. Apply

For TBSY, apply on the official portal or through the hospital or thalassemia society. For PMJAY-MA, the hospital sends the pre-authorisation. For relief funds, follow the office's process.

8

Step 8. Follow up

Keep copies of everything, note the application number and track the status on the portal. Ask the hospital's social worker or patient coordinator for help.

9

Step 9. Plan for the gaps

If the estimate is higher than the approved amount, begin parallel plans early, such as NGO help, relief funds and crowdfunding, so the transplant is not delayed.

Common mistakes to avoid

  • Waiting too long. Early transplant gives better outcomes and the scheme has had preferences for younger children.
  • Not doing HLA typing early.
  • Relying on an old hospital list or an agent. Use only official portals.
  • Paying anyone for "guaranteed approval".
  • Forgetting post-transplant costs. Budget for medicines, tests and stay for at least two years.
Critical Care Preparedness

Blood & Platelet Readiness During BMT Conditioning

Many families focus entirely on surgical funding and are caught unprepared during the hospital conditioning phase. In the 2 to 4 weeks following high-dose chemotherapy or conditioning therapy, the child's bone marrow stops producing new blood cells entirely. During this intense cytopenic period, patients require:

  • Irradiated Packed Red Blood Cells (PRBCs): Specially irradiated blood to maintain safe haemoglobin levels without triggering transfusion-associated graft-versus-host disease (TA-GvHD).
  • Single Donor Platelets (SDP): Apheresis platelets required every 24 to 48 hours to maintain platelet counts above safe thresholds and prevent life-threatening spontaneous internal bleeding.

Don't scramble for emergency donors in the ICU.

Frequently asked questions

Is BMT for thalassemia free in India?

Not automatically. Eligible children can get up to 10 lakh rupees through TBSY, some states and hospitals offer free or subsidised transplants, and PMJAY-MA may cover costs in Gujarat depending on the package. Confirm with your hospital.

What is the age limit for TBSY?

According to the government information page, thalassemia patients up to 12 years of age are covered. Check the portal for any changes.

Can I apply if I do not have a matched sibling?

Rules differ by scheme and phase. The original TBSY guidelines needed a matched related donor, but recent public information mentions separate eligibility for matched unrelated donors. Ask the transplant centre and check the portal.

Is the 10 lakh paid to the family?

No. The funds are paid directly to the hospital performing the transplant.

Where can I apply for TBSY?

Through the official TBSY portal on the Coal India website, or through participating hospitals and thalassemia societies.

Conclusion

A bone marrow transplant can free a child from a lifetime of transfusions, and financial help does exist. The most important steps are to act early, get HLA typing done, choose an empanelled hospital, keep your documents ready and apply through official channels. If you need blood for a thalassemia patient, or want to become a regular donor, visit LifeSavers United's Request Blood and Donor Registration pages.
